Blue Basic Co-pay: This plan offers office visits, and preventive care, in addition to hospitalization and emergency care for a $30 co-pay. Prescription drug coverage involves a 30%co-insurance and a separate deductible. There is no supplemental accident coverage with this plan, but there is accidental death benefits and maternity after a twelve month waiting period.
Blue Advantage: This plan offers the same basic coverage as the Blue Basic, except the co-pay amount is $20 co-pay per service, there is a 20% co-insurance, and there is no deductible required for prescription drug coverage. Families can choose a deductible ranging from $750-$2000.
Blue Basic Co-insurance: This plan offers most medical coverage, including hospital in-patient and outpatient care, and well-child office visits, for a 30% co-insurance. Accidental death benefits are available, as is maternity after a 12-month waiting period.
Blue Advantage Co-insurance: Identical to the Blue Basic Co-insurance, this plan carries with it a 20% co-insurance instead of 30%. Families choose deductibles between $5,000-$15,000.
