QualChoice offers three basic ways to ensure your family's health: their PPO, HMO and POS plans. In their PPO (preferred provider organization) plan, customers are free to choose from any participating physician to receive maximum coverage. In their HMO plan, customers are paired with a primary care physician to coordinate their care and refer them to specialists if necessary. On their POS (point-of-service) plan, you choose where you want to seek care- from your primary care physician, any in-network doctor, or from out-of-network doctors. Here are some details of plans available from QualChoice for individuals and families:
* HMO Plan - In this plan, you are paired with a primary care physician who oversees your health and any specialist visits you may require. Covered benefits include: preventative care visits, such as well-child mammograms and cervical exams, physician services including specialist services; outpatient services such as emergency clinics, inpatient hospitalization services, including surgery; rehabilitative services and selected physical therapies.
* PPO Plan - In this plan, you are free to see any physician you like, with the highest level of benefits available only when you see network physicians. Benefits are similar to QualChoice's HMO plan.
*POS Plan - The POS plan gives you the options of an HMO, a PPO and a traditional indemnity plan, all in one. Option One has you choose a primary care physician. This option brings you the lowest out of pocket cost. Option Two lets you choose any in-network physician, still getting a reduced network rate. Option Three covers you for any licensed physician, but your out-of-pocket cost is higher. No matter which option you choose, you are covered for major medical and regular doctor visits, along with certain out-patient services.
