Family Health Insurance Options from PacifiCare of Oregon (OR)
Their coverage plans vary in deductible and premium costs, leaving no one out of the mix. Plans currently offered to families include:
Pacificare Signature Value HMO: With this HMO plan, members pay a small copay for most services, not to exceed $15,000 per year per family, including in-patient and outpatient care, well-visits for children and preventive care for adults, and routine office visits. Maternity services may be available. Prescription drug coverage is offered on a co-pay basis.
PacifiCare Signature POS: This POS plan boasts the best of both worlds. For a small copay, choose network doctors for such things as preventive care, well-visits, outpatient care and hospitalization. If you don't mind spending a bit more, than you can choose your own doctor and pay a higher co-pay.
PacifiCare Signature Options PPO: Once you meet the annual deductible of $5,000, you pay a predetermined co-insurance amount, usually between 10%-30%, for services such as well-child and office visits, hospitalization, outpatient care and preventive care. You are free to see any doctor without a referral. Prescription drug coverage is not available with all plans.
