Pacificare Signature Value HMO: With this HMO plan, members pay a small copay for most services, including in-patient and outpatient care, well-visits for children and preventive care for adults, and routine office visits. Maternity services may be available. Prescription drug coverage is offered on a co-pay basis.
PacifiCare Signature POS: This POS plan boasts the best of both worlds. For a small copay, choose network doctors for such things as preventive care, well-visits, outpatient care and hospitalization. If you don't mind spending a bit more, than you can choose your own doctor and pay a higher co-pay.
PacifiCare SignatureIndependence (Indemnity): Once you meet the annual deductible with this plan, you pay a predetermined co-insurance amount, usually between 10%-30%, for services such as well-child and office visits, hospitalization, outpatient care and preventive care. You are free to see any doctor without a referral.
