There are a range of deductible options in the SuperMed One plans, including a high-deductible option meant for use in conjunction with a health savings account (HSA). Here are some details of Medical Mutual's SuperMed One plans:
* SuperMed One 500 - Annual deductibles on this plan are $500 for individuals and $1,000 for families. This plan covers regular doctor visits and urgent care visits at 100% after a low copay of $15. Other covered services carry a 20% coinsurance cost, and include well child care, routine gynecological services, allergy treatments, adult immunizations, diagnostic services, hospitalization and prescription drug coverage. Services outside of their provider network carry a higher coinsurance cost.
* SuperMed One 1000 - Annual deductibles on this plan are $1,000 for individuals and $2,000 for families. Coverage is identical to SuperMed One after the deductible is met, including the prescription drug plan.
* SuperMed One $2,500 - Annual deductibles on this plan are $2,500 for individuals and $5,000 for families. This plan covers all services 100% after copay, including regular doctor visits, specialist visits, urgent care services, well child care, routine gynecological services, allergy treatment, adult immunizations, diagnostic services, hospitalization and prescription drug coverage. Services received outside their care network carry a 40% coinsurance cost.
