Unfortunately, Lifewise Health Plan of Arizona, whose parent company has been serving the health insurance needs of the Western United states since 1933, no longer offers health insurance coverage to residents of Arizona. As of February of 2009, Lifewise discontinued operations in the state, yet continues to operate in other states in the U.S.

LifeWise Passports 20: Five deductible options for families, ranging between $1500-$15,000. Office visits, preventive immunizations, emergency care (with a co pay), acupuncture, mental health and nicotine treatments were covered. There was a separate prescription benefit add-on available.
Lifewise Essentials 20: There were two deductible options with this plan. With a 20% coinsurance for most services, it covered preventive and office visits, prescription drug benefits, inpatient care, and nicotine addiction treatment. Emergency care was covered with a $100 co-pay after the deductible was met.
