Personal Advantage plan: This plan includes outpatient visits, hospitalization, preventive care, maternity, prescription drug coverage and emergency care. Some deductibles will apply.
Co-pay plans: These plans offer a no-deductible with no lifetime maximum benefit. Office visits for children two and under are free. In addition, maternity coverage, pre-natal care, eye exams, urgent care, and prescription drug coverage is available for just a co-pay. The platinum plan without Rx coverage has a $100 co-pay for emergency room coverage.
Gold 500/1000: This deductible plan has unlimited benefits. Among covered services are outpatient care, laboratory, x-ray and hospitalization; There is an optional rider for prescription drug coverage.
Silver 1500/2500/3500: This plan is similar to the Gold plan, including the outpatient coverage and office visit co-pay. The deductible is waived for well-baby services, and after the hospitalization deductible is met, co-insurance is 20%.
