50 Co-pay plan: Among benefits include a well-child, eye exam and outpatient co-pay. There is no prescription coverage with this plan.
25 Co-pay plan: Pharmacy coverage involves a $250 deductible; well-child, eye exams and outpatient care is covered at no charge.
250/1500 Deductible: Members have their choice of deductibles; family deductibles are twice that of the individual. Pharmacy coverage is available with a separate deductible of $250 per year. Well-child care, vision and hearing screenings, scheduled pre natal visits, outpatient x-rays and immunizations are covered with no deductible. Hospital coverage is $500 after the deductible.
