HealthPartners Individual Plan: This plan offers families eight deductible levels. Routine physical and eye exams are covered after the deductible is met. Well-child is covered 100% to age six, shots to age 18; Office visits and pre natal visits are covered 80%, although if you would like to take advantage of their maternity coverage there is an 18-month waiting period. Dental and Prescription drug coverage is optional, and emergency service and hospitalization (including emergency dental procedures) is standard.
