Their coverage plans vary in deductible and premium costs, leaving no one out of the mix. Plans currently offered to families include:
Pearl 25 HMO: There is no deductible and an unlimited maximum benefit with this plan. Well-child visits are offered with $25 co-pay. Also offered are hospital, preventive care, outpatient, chiropractor, massage and prescription drug coverage. $250 in pregnancy costs are also covered for those enrolled in the program.
Diamond 15: This plan offers six deductible choices, and includes office visits, well-visits, x-rays, gynecological and preventive visits with a co-pay. Co-insurance is used for accident coverage, maternity and hospitalization. Prescription drug coverage is also available.
Emerald 40: This plan has a 30% co-insurance for hospitalization and 40% co-pay for well-visits. This plan also offers a prescription drug plan. Preventive vision, routine physicals and well-baby visits are not covered.
Garnet 50%: Members choose between a zero or $10,000 deductible. They pay 50% for most services, including well-baby visits, hospitalization, and office visits. There is no preventive medical package, prescription or accident coverage.
