Excellus offers HMO (health management
organization) plans for individuals and families at
lower cost than comparable coverage elsewhere. In these
plans, customers are assigned to a primary care physician
to coordinate their care at participating facilities.
There are a range of plans and plan options to choose
from, including:
* Healthy NY - This
plan is their most affordable plan for individuals
and families. After meeting certain income and employment
criteria, customers get major medical benefits at
a reduced cost. Copays and some coinsurance charges
apply, after which you get access to: hospitalization
benefits, well-patient benefits, out-patient benefits,
nursing care and services and optional prescription
drug benefits.
* Family Health Plus
- This plan includes visits with your primary care
physician, who can refer you to other covered care,
such as diagnostic services, surgery, OB/GYN services,
maternity services, occupational therapy or prescription
drugs.
Other Plan Options
In addition to their HMO plans for adults,
Excellus offers insurance directly to children through
their Child Health Plus plan. This plan offers full
coverage to children, regardless of whether or not their
parents are members, at a lower cost than comparable
coverage for an adult.
