• Preferred-Care Blue Premium PPO: With deductibles ranging from $1500-$15000 and f our rates to choose from, this PPO plan works well for the budding family. Included in the package is a co-pay for prescriptions as well as an option between co-pay or co-insurance for other services.  Covered immediately are well visits, hospitalization, life insurance, dental care, eye wear discounts and allergy testing. Maternity coverage is subject to a two-year waiting period.
  
  
• Preferred-Care Blue RateSaver: This plan is slightly limited on some of the common coverage items. It does not offer maternity, prescription or well child/preventive service; however, it does have a $5000 term life insurance policy attached to it, as well as eyewear discounts and script-saver, a prescription discount program.
  
  
• Blue Care HMO/Blue Care Select HMO: There is no deductible with this HMO plan. Its concentration is in preserving wellness. It offers wellness care, prescription coverage with a co-pay, vision, hearing and life insurance, as well as maternity coverage.
