individuals and families:
BlueOptions: This PPO
plan offers full coverage with a range of financial
options, including one designed to take advantage of new
health savings accounts (HSAs). Deductibles and percentage
of coverage can be matched to the amount customers are
looking to pay annually versus the amount they want
to pay out of pocket per visit.
BlueChoice PPO: This
PPO has similar benefits to BlueOptions. BlueChoice
customers choose a primary care physician to coordinate
their care, after which they can see any additional
specialist within the PPO network without a referral.
BlueCare HMO: The BlueCare
HMO offers complete coverage with low, predetermined
copayments and no deductibles. Choose a Primary Care
Physician from the Health Options network and enjoy
a wide range of coverage, from prescription drugs to
wellness visits and immunizations.
Traditional Dimension IV Plan
(DIV): Dimension IV is a traditional indemnity
plan which allows you to see any doctor you like, in
or out of network. Deductibles range.
DIV has the added option of sole coverage for a child
3-17, regardless of whether or not the parent has a
Dimension IV plan.
