Assurant Health has been in business since 1892. The formation of Time Insurance Company to expand their coverage area has led to a company which serves over one million people. The carrier offers families plenty of options when it comes to keeping their loved ones healthy. Their coverage plans vary in deductible and premium costs, but all of them offer unlimited ICU and worldwide coverage.
One Deductible Plan PPO: Families have three different deductible options. Once met, the plan pays 100%. Prescription plan is offered, as is well-child up to $500 maximum per year. There is no supplemental insurance or maternity with this plan.
One Deductible Plan Traditional: There is only one deductible for all persons covered. No network to worry about. Wellness is covered up to $500, and this plan has a prescription plan. No maternity coverage.
PPO X-tra Plan: A high family deductible offers low monthly premiums. Prescription, maternity and Doctors' visits are covered (with a co-pay). Co-insurance is 20% after the deductible has been met.
Traditional Plan: This has a standard deductible of $2500, with prescription, well-child and preventive medical included. Maternity is optional.
Value Plan: Maximum deductible is $5000, with prescription, well-visits, optional maternity and a co-insurance varying between 50-80%.
